Utility Waiver Form
It is mandatory by state law to get all underground utilities marked before staking into the ground.
*Do not wait less than 5 days before your delivery date to schedule markings!
*All OCCASION WILL NOT SET UP TENTS OR INFLATABLES WITHOUT UTILITIES MARKED!
What you should know:
1. Renter (You) is responsible for getting the utilities marked.
2. Tents and inflatables require underground utilities to be marked.
3. Renter must mark all septic or sprinkler systems themselves (811 will not do these).
4. 811 should mark your space within 48hrs of calling, if not call them ASAP!
5. 811 does not let you know when they arrive or leave the property, look for marks in the yard.
What you will need to know prior to calling for locates (MUST read below before calling!)
1. Phone and address of All Occasion Party Rental. (listed above)
2. Event site address and where on property digging/staking is taking place.
3. Contact person’s name and phone number of delivery and set up site (renter).
4. What type of digging is being done (setting up a tent or inflatable).
5. The stakes will go 3ft (36”) into the ground.
6. The township and county of delivery site (see 811 website for township help).
7. Start date (is your scheduled delivery date).
8. End date (is your scheduled pickup date).
